Prevention, Health Promotion, and Social Work: Aligning Health and Human Service Systems Through a Workforce for Health

Access to and quality of health care account for onhy 20% of a person's health and welk-being; the physical
erwironment, social determinants, and benavioral factors account for 80% of health outcomes. The National Acade-
mies of Science, Engineering and Medicine (NASEM) report states, “The health sector must decide whether to build
capacity to dieciiy assist patients! nonmedical needs or, altematively, to partner wilh community-based organizabons
[5] that have experience in addressing social needs and the necessary capacity to address those needs.” We contend
that both oi these activities are possible and that both will be required to improve our nation's health. Likewise. an ex-
panded definition of the healh sector that not only refrains from bifurcating social and heallh servica dekvery systems
but also reconceptualizes membershãp in the existing health workoroa is paramount to transformalion.
The NASEM report defines the composition of the traditional health care workforce as providers of medical
[10] care (i.e., physicians and nurses), it also acknowiedges critical contributions of the social care workforce in improving
patient hesith outcomes. Across the world, heaiih systems are increasingly recognizing that keeping people heslthy
and aloving them to tivive require more than access to and delivery of recommended medical care in large hospital
settings. As acknowiedged in the report, research increasinghy points to lhe need for health care service delivery to
shif from hospital to communiy-based settings. Refocusing on community-based care requires a prepared workfor-
[15] oe that can be deployed in nontradiional hsaih care settings with the skils needed to navigate community dynamics,
resources. and linkages. Regardiass of health seting. effectively addressing social determinanas of health (SDOH)
requires a skillad workforce prepared lo engage and inlervene with individuals, families, and communities; mobilize
resources; and work across systems. As a profession, social workers have long been experts in these aclivibes, the
profession has its origins in addressing one of the most potent and seemingty iniractable SDOH-—poverty—and has
[20] been bridging social and heaiih care systems for more than 100 years.
By the nature of their knowledge base and skill set acquired through masters-level training groundad in per-
sor-in-environment and ecological systsms approaches, social workers ars experts in the delivery o! social care.
The social work woriforce is well versed in how individuais, familias, and communítes are shaped by thai environ-
ments. People do not operate in isotaton, rather, they are influenced by the pirysical and social ecology in which they
